Forms and use of your data
This page explains in plain language what information is collected when you use the MHSV® website forms, why it is used and where you can find further information.
Fields marked with an asterisk (*) are mandatory. All other fields are optional.
Please do not provide sensitive personal data, particularly medical or health-related information, in open-text fields unless MHSV® specifically requests it through an appropriate and secure process.

Data collected:
- first and last name;
- email address;
- telephone number, if you choose to provide it;
- profile or organisation;
- subject and content of your message;
- certain technical information required for the operation, security and protection of the form against misuse.
This information is used to identify and respond to your enquiry, direct it to the appropriate person within MHSV® and provide the necessary follow-up.
3. Premium Founding Book order request
Data collected:
- requested edition and quantity;
- first and last name;
- organisation;
- country;
- email address;
- telephone number, if you choose to provide it;
- any additional message you choose to provide;
- certain technical information required for the operation and security of the form.
This information is used to review and process your request, contact you, confirm the Book’s availability and applicable arrangements, organise its potential collection or delivery and manage the related administrative follow-up.
Submitting this form constitutes an order request only. It does not represent payment or automatic confirmation of an order. MHSV® will contact you separately to confirm the applicable arrangements.
4. Newsletter subscription
Data collected:
- email address;
- first name, if you choose to provide it;
- preferred language;
- information required to record and manage your subscription.
This information is used solely to send you the MHSV® newsletter and information about our activities, projects, events and news.
Your subscription is based on your consent. You may unsubscribe at any time using the link included in each message. Your email address will then be removed from the active mailing list.
5. Recipients, retention and security
Your information is accessible only to authorised persons within MHSV® and, where necessary, to technical service providers involved in operating the website, email services or newsletter, including Brevo.
MHSV® does not sell your personal data.
Your information is retained only for as long as necessary to process your request, manage your subscription or comply with applicable legal, contractual and administrative obligations.
6. Your rights and further information
Depending on the applicable law, you may request access to, correction or deletion of your personal data, withdraw your consent where processing is based on consent or object to certain processing activities.
